Forms and Material for Clinical Directors

For instructions on how to conduct a screening, download the Special Olympics Fit Feet Training Manual (625K PDF file, updated June 2006). This manual was authored by the Global Clinical Advisor for Fit Feet:

The following forms are designed for the use of Clinical Directors when administering their individual programs:

  • Hold Harmless Agreement (Adobe PDF, 124K): All health care providers volunteering to conduct health screenings at Special Olympics Healthy Athletes events must have malpractice insurance that covers the Healthy Athletes related activites. This form's mutual hold harmless language is a method by which each party is protected.

  • The Special Olympics Fit Feet Prescription/Screening Results form (report card) is designed to print two-up on an 8½" x 11" sheet of paper; cut the paper in half horizontally to create two 8½" x 5½" forms for each sheet printed. Download report card (Adobe PDF, 36K)

A series of pamphlets have been developed with Special Olympics athletes in mind — see brief descriptions below. Lighthearted illustrations are used to provide guidance on general foot care as well as on specific toenail and skin care. Copies of the three Fit Feet brochures are available by e-mailing devangelista@specialolympics.org, or they are available to download (Adobe PDF format):

General Care
A general guide on foot care, e.g., cleanliness, grooming, protection and appropriate fit of shoes.

Skin Care
Provides skin care for feet by asking and answering common questions such as what to do if you have a blister or if your feet is sore.

Nail Care
A guide to common questions dealing with nail care, illustrates how to maintain nails properly and gives examples of what not to do.
